She is said to have undergone breast augmentation surgery through an armpit incision in another country eight years ago. At first, she enjoyed having larger breasts, but as time went on, she noticed sagging and found the large size burdensome. Physical examination
The physical measurement and 3D scan showed that the breast size are about D-cup to E-cup. The previous textured implants was not fixed and moved around, but fortunately, there was no capsular contracture.

Why do the breasts appear saggy?
Despite the nipple position being considered normal, the breasts appear saggy due to the downward displacement of breast volume. Despite having implants, the volume is disproportionately distributed downwards, resulting in inadequate upper breast volume and breast sagging.
Therefore, surgery should be performed to redistribute this volume balance upwards. The best way to achieve this is through glandular repositioning. In the case of glandular repositioning performed after implant removal, the technique I described in the previous post, involving reshaping the upper pole, can be applied.
Which incision method would be best to choose?
The most important point to consider when choosing the incision method is the skin on the lower pole that has been stretched. After removing the implants, she desired a smaller and perkier shape. Here, the crucial aspect is to appropriately reduce the stretched lower skin to achieve satisfactory results.

In this case, the most reliable method is the inverted T incision technique. The stretched skin is directly excised both vertically and horizontally, resulting in a long scar in the shape of an inverted “T”. However, a drawback is that the inframammry fold may not rise and remains fixed, which can be disadvantageous for breast lifting.
Another option is the verticval incision technique. The advantage over the inverted T incision is that it leaves shorter scars without submammary scar and can enhance the lifting effect by raising the inframammary fold. However, a potential drawback is that inexperienced surgeons may cause the skin in the lower pole to pucker or become uneven, resulting in an awkward breast shape. Nentheless, these drawbacks can be adequately prevented.
